vellus hair into thick, pigmented terminal hair in androgen-dependent body regions. Hirsutism affects approximately 5-10% of women of reproductive age. It can be the sole sign of an increase in androgens (male hormones) or it can occur alongside conditions like acne and androgenic alopecia. Structurally, there are three types of hair: Lanugo hair is the soft, non-pigmented hair covering a fetus’s body, which typically disappears within the first months after birth. Vellus hair is slightly larger, non-pigmented, soft hair found in body areas not dependent on androgens. Terminal hair, on the other hand, is long, pigmented hair. In women, hirsutism is characterized by excessive and coarse hair growth in areas typically associated with men (such as the mustache and chin area on the face, chest, back, abdomen, buttocks, and inner thighs). This condition can significantly impact a woman’s quality of life and self-confidence. While body hair varies from person to person and by ethnic background, hirsutism manifests as a distinct increase in dark and stiff hair. Hair growth in the armpits and genital area may also show a male-pattern distribution.

What Are the Causes of Hirsutism?

The female body naturally contains small amounts of male sex hormones (androgens). However, an increase in androgen levels or an overreaction of hair follicles to these hormones can lead to hirsutism.

The most common and important causes of hirsutism include:

Polycystic Ovary Syndrome (PCOS): This hormonal disorder is responsible for the majority of hirsutism cases (up to 80%) and leads to increased androgen production in the ovaries. PCOS can also present with additional symptoms like menstrual irregularities, acne, and hair loss.

Congenital Adrenal Hyperplasia (CAH): Congenital adrenal hyperplasia is a genetic disorder caused by a mutation in the CYP2A2 gene, leading to decreased 21α-hydroxylase enzyme activation. Unlike the classic forms, such as the simple virilizing and salt-wasting forms, late-onset congenital adrenal hyperplasia (LOCAH) presents with hirsutism and an irregular menstrual cycle starting at menarche. In LOCAH, hirsutism, menstrual irregularities, or primary amenorrhea occur without signs of adrenal insufficiency. Depending on racial and genetic differences, it accounts for 1.6-10% of hirsutism cases. The diagnosis of LOCAH does not change the approach to hirsutism management but is important for genetic counseling and fertility considerations.

Cushing’s Disease: ACTH-secreting pituitary adenomas are a rare but significant cause of hyperandrogenism. Besides the symptoms of Cushing’s disease, such as central obesity, buffalo hump, striae, and easy bruising, hirsutism occurs in 80% of cases. In addition to hirsutism, acne, male-pattern hair loss, and menstrual irregularities may also be present.

Severe Insulin Resistance Syndromes: In severe insulin resistance syndromes, ovarian androgen secretion increases via insulin receptors and IGF-1 in the theca cells. In the presence of hyperinsulinemia, SHBG levels decrease. Although total testosterone levels may be normal, free testosterone levels can be elevated. Genetic defects in insulin receptors, insulin receptor antibodies, lipoatrophy, and lipodystrophies cause insulin resistance.

Idiopathic Hyperandrogenemia: This is a condition where clinical and biochemical signs and symptoms of hyperandrogenism are present, but normal ovulatory cycles and normal ovarian structure exist. In some women, there may be an increase in steroid activity despite normal serum androgen levels.

Iatrogenic Hyperandrogenemia: Anabolic androgens can be used to enhance athletic performance, increase libido, or for cosmetic body shaping. These orally, parenterally, or transdermally used anabolic androgens lead to acne vulgaris, hirsutism, striae, and seborrheic cutaneous changes.

Idiopathic Hirsutism: A condition in which there are no clinical or laboratory signs of hyperandrogenism other than hirsutism, and normal ovulatory cycles and ovarian structure are present, is referred to as idiopathic hirsutism. It accounts for about 5-20% of women with hirsutism.

Medication Use:

Certain hormonal therapies (e.g., progestogen-containing contraceptives).

Medications containing male hormones, such as anabolic steroids.

Genetic Factors and Hereditary Diseases:

Metabolic disorders that cause an imbalance in the body’s hormone production, such as congenital adrenal hyperplasia.

Increased hair sensitivity due to ethnic background can be observed in some families.

Hormone-Producing Tumors:

Though rare, tumors in the adrenal glands or ovaries that produce excessive male hormones can lead to hirsutism. In such cases, hair growth often starts suddenly and increases rapidly.

Obesity: Excessive weight can affect the body’s hormone balance and contribute to an increase in androgen levels.

Notably, in women with mild symptoms (up to 50%) or in 20% of those with severe hirsutism, no clear cause may be identified.

What Are the Symptoms of Hirsutism?

Hirsutism in women typically manifests as excessive growth of thick and dark hair in male-pattern body regions. These therapies usually require long-term use (at least 6 months), as their effects become apparent over time.

Oral Contraceptives (Birth Control Pills): Birth control pills containing estrogen and progestin work by reducing androgen secretion from the ovaries and lowering free testosterone levels.

["post-2157","page","type-page","status-publish","has-post-thumbnail","hentry

Hirsutism - Uzm.
